Preparing for your Initial Consultation

To prepare for your initial consultation, please bring a list of any medications you are currently taking. Additionally, please provide details of where recent scans and blood tests have been performed.

If you have received a demographics form, we recommend bringing it with you to ensure that your information is accurate in our systems. This will help our administration staff provide you with efficient and effective support throughout your treatment journey.
